I couldn't really see a way around it. If he gives, he bleeds out, and dies. If he stops the blade, he bleeds out and dies. A hole in your stomach is a slow death, and would be hard to close up in the conditions laid out. So to keep some realism, a delusional coma seemed the route, and a more fun outcome.

Well, he had a walkie talkie. He could have just called for someone to come with medical help. They'd pick him up in a helicopter and give him medical help on the way to Utopia.

I had him break it so he wouldn't be interupted during the fight, so he would have been screwed in that case.
